Care Coordination is a process that we at Orlando Premier Psychiatry Behavioral Health pioneered as part of our comprehensive approach to your mental health care plan.
Many initial appointments begin with a conversation with one of our Care Coordinators. The purpose of this initial meeting is to assess your needs and help guide you to the services that will help you reach your treatment goals and better mental health.
How Does Care Coordination Work?
Orlando Premier Psychiatry Behavioral Health is staffed with a team of knowledgeable, experienced intake specialists. The goal of Care Coordination is to support you during the beginning stages of your care with Orlando Premier Psychiatry.
Care Coordinators serve as your first point of contact, answering any questions you have while providing you with the information you need to get the most successful results from your treatment. Our Care Coordination team is familiar with all the treatment options available at Orlando Premier Psychiatry Behavioral Health and will match you with the right provider (or multiple providers) for your tailored care plan.
Can Care Coordination Help Me?
In many cases, we find that most people benefit from working with their Care Coordinator. Care Coordination offers an individual approach to ensure you can find the right mental health care for you.
It is important to us that you feel safe and comfortable with your Care Coordinator and with all the providers recommended to you. Bring a list of questions so your Care Coordinator can focus on addressing any concerns you have.
What Can I Expect From Care Coordination?
At your first meeting, your Care Coordinator will take a medical history, much like your first appointment with a medical doctor. Your Care Coordinator will ask you about any over the counter or prescription medications you’re currently taking.
They can help you complete any necessary paperwork to get you ready for your first appointment.
Asking for help is a big step, and while it can be exciting, it can also feel overwhelming. Your Care Coordinator will take the time to understand you and will ensure you get the support you deserve. You will leave your intake appointment with appointments scheduled so that you know what your next steps are.
